Moving Your X-Rays and Dental Records
If you are coming to our office for the first time, we would like to obtain X-rays from your previous dentist, especially if they were taken within the past three years.

We have a legal obligation to take and maintain current dental X-rays in order to fully diagnose our patients’ oral health. In fact, we are not able to treat patients who refuse the minimally acceptable diagnostic dental X-rays.
If previous X-rays cannot be obtained, if they are out of date, or if they are not of acceptable diagnostic quality, you must have diagnostic X-rays taken as a new patient in our office, often at your first dental exam.
Use the records release form linked on this page.
Fill in your details and sign the release.
Scan and mail or email it to your previous dentist. It serves as your records request.
We adhere to the Dental Practice Act standard to process the requests we receive. The estimated turnaround time is 5 business days for electronic requests and 10 business days for printed requests.
State Board of Dentistry regulations require that record reproduction in a private office be provided either free of charge or for a “reasonable” fee reflecting the cost of reproduction.
