Dental Memberships in Collegeville, PA

This is an in-house dental membership plan offered directly by Advanced Dentistry of Collegeville. It is not dental insurance, not a health insurance plan, and does not satisfy any requirement to obtain health insurance.

Membership benefits and discounts are valid only at Advanced Dentistry of Collegeville and may not be combined with dental insurance benefits, third-party discount plans, insurance-network discounts, or other promotions unless the Practice approves otherwise in writing.

Membership is non-transferable and may be used only by the enrolled Member. Services are subject to the dentist’s clinical judgment, the Member’s ongoing eligibility, and the Practice’s standard policies.

Discounts apply to the Practice’s usual and customary fees at the time treatment is rendered unless otherwise stated in a written treatment plan. Discounts are not retroactive and cannot be applied to services completed before enrollment or after membership expiration or termination.

Members remain responsible for laboratory fees, specialist fees, outside referrals, prescription costs, and services not specifically included in the membership agreement unless otherwise stated in a written treatment plan.

Your initial membership term begins on your Membership Start Date and continues for 12 months.

Membership automatically renews for successive 12-month terms unless cancellation notice is provided at least 30 days before the applicable renewal date.

By enrolling and providing a credit card, the Member authorizes the Practice to charge the card on file for applicable monthly membership fees or the annual membership fee upon renewal, depending on the payment option selected.

Members may cancel by providing written notice at least 30 days before the annual renewal date. Notice may be provided in person or by email.

Cancellation becomes effective at the end of the current membership term. Membership fees already paid are non-refundable, and unused benefits do not carry forward.

If a membership is cancelled, expires, or lapses and the Member later chooses to re-enroll, a $50 reactivation fee applies in addition to the then-current membership fee.

The Practice may terminate membership for nonpayment, repeated missed appointments, abusive or inappropriate behavior, fraud, material misrepresentation, or failure to comply with Practice policies.

Membership does not guarantee treatment, clinical outcomes, appointment availability, or coverage for every dental service a Member may need. Treatment recommendations are based on the dentist’s clinical judgment and the individual needs of each patient.

If periodontal maintenance is required instead of a routine prophylaxis, the periodontal maintenance discount applies and the routine cleaning benefit does not apply to that visit.

Implants, crowns, dentures, partial dentures, orthodontics, and other extensive treatments may require separate treatment plans, deposits, laboratory charges, financing arrangements, and informed consent documents. Membership discounts apply as stated in the final treatment plan.
