When you call to schedule, please have your dental and health insurance information ready, along with the name of the employer and the policyholder if your plan is employer sponsored. Medicare, Medicaid and health insurance details are most often needed for our sleep apnea patients. If your coverage has changed, please send us your new insurance information.
